Phases 1-3

Phase 1 (First 90 - 120 Days)
⦁ Complete Individual Recovery Plan as defined and agreed upon during assessment
⦁ Complete IOP if prescribed
⦁  Completed Steps 1 – 8 with Designated Sponsor and are making 9th Step amends
⦁ Join 12 Step Home Group
⦁ Minimum Five 12-Step Meetings per Week
⦁ Meet with Sponsor 1x Per Week
⦁ Weekly Life Skills Development Classes
⦁ Daily Prayer and Meditation
⦁ Weekly One-on-One’s with Case Manager/Recovery Coach
⦁ Develop and Maintain a Healthy Diet
⦁ Participate in Group Recreational & Social Activities
⦁ Look for and Secure Employment
⦁ Follow All Program Guidelines
⦁ Apply for Senior Residency & Phase 2

Phase 2 (4 months - 1 Year)
⦁ Actively working Steps 10, 11 & 12; substantial progress made towards completing 9th Step
⦁ Actively Participate in Home Groups
⦁ Minimum Four 12-Step Meetings per Week
⦁ Meet with Sponsor 1x Per Week
⦁ Weekly Life Skills Development Classes
⦁ Attendance at Aftercare, if prescribed
⦁ Daily Prayer and Meditation
⦁ Meet with Case Manager/ Recovery Coach as prescribed
⦁ Maintain Healthy Diet
⦁ Begin Physical Fitness Routine
⦁ Participate in Group Recreational & Social Activities
⦁ Gainfully Employed, In School or Participating in Career Development Training
⦁ Follow All Program Guidelines
⦁ Develop a Transition Plan with Staff, which may include moving to Next Step Apartments

Phase 3 (1 Year – 2 Years @ Next Step Apartments)
⦁ Actively working the 12th Step
⦁ Sponsoring PHouse residents in Phase 1 program
⦁ Meet with Sponsor 1x Per Week
⦁ Minimum Three 12-Step Meetings per Week
⦁ Continue Physical Fitness Routine
⦁ Maintain Healthy Diet
⦁ Follow All Program Guidelines
⦁ Develop a Transition Plan with Staff
⦁ Gainfully Employed, In School or Participating in Career Development Training
